The paradox of this series, likening the treatment process of an addictive disorder to the solving of a Rubik’s Cube, is that the last side of the cube is the only one that really matters. If we never reach this point in treatment, the client’s hope of a quality future is minimal.

Despite their advanced training, physicians are not immune to the perils of addiction. The prevalence of substance use disorders among physicians is similar to that in the general population, and substance-related problems continue to be among the chief reasons physicians are referred to their licensing boards for disciplinary action (Flaherty & Richman, 1993; Brewster, 1986; Hughes, Brandenberg, Baldwin, et al., 1992).

Mood disorders account for the vast majority of psychiatric disorders that occur in the world. In fact, according to the Global Burden of Disease study, which analyzed the impact of 107 major diseases on morbidity and mortality, mood disorders are currently the world’s most disabling condition (Ustun & Kessler, 2002).
In substance abuse treatment facilities and the addictions field, in general, clinicians provide treatment to a significant number of patients dually diagnosed with both a substance abuse disorder and a co-morbid mood disorder.

Chronic Addiction and Recovery Management: Implications for Clinical Practice

In 2001, Dr. Lonnie Shavelson published Hooked, a provocative study of the lives of five individuals whose addictions propelled them through multiple episodes of addiction treatment and encounters with a host of other community institutions. Shavelson’s ethnography of addiction was, in part, an exposé of a treatment system that failed to comprehend and respond to the chronic and complex nature of addiction and recovery. This article summarizes the empirical evidence that addiction is, for many, a chronic condition; identifies risk factors for such chronicity; highlights the consequences of treating chronic addiction in the context of an acute care model; and notes the emergence of sustained recovery management models as well as the clinical implications of such models.

A recent New York Times article by Charles Siebert describes an increase of violence among young male elephants. Siebert states that studies of elephant behavior reveal that young male elephants have been dramatically affected by witnessing the massacre of their communities/families, the death of a parent, the loss of elephant elders, particularly bull male elephants, who traditionally socialize young male elephants by modeling how to handle aggression properly. These losses occur at the hands of poachers, who kill elephants for the purpose of seeking greater land space and money from the sale of the elephants’ ivory (Siebert, 2006).
