Our field has been going at dual disorder problems for about 20 years now, and in this day and age, we are constantly calling for evidence-based treatment to support what we do. So, it seems about the right time to ask just how effective are the existing dual disorder treatments we throw at our clients with mental health and substance abuse problems.

It was 1995 and a man was walking around with a wood block in his shirt pocket. Occasionally he would take it out and fiddle with it as if he were checking appointments. Some might have thought him crazy, but he was not.

The man was Jeff Hawkins, and he would soon revolutionize the technology industry with a device known as a Personal Digital Assistant, or PDA. It would be called the Palm Pilot and sell over one million units in its first 18 months alone.

I suppose the members of each generation reach an age where they come to the conclusion that the world has changed dramatically within the brief number of years since their birth. The changes seem to present themselves overnight, typically as some new technology is finally embraced by an ecstatic public that never previously recognized the possibilities.

Sally was sitting in front of me reluctantly rolling up her sleeve to show me the razor cuts she had made on her arm the night before. Although she had attended and actively participated in an Alcoholics Anonymous meeting that night, she still cut herself to alleviate the unbearable emotional tension she was experiencing.

For two years I had been treating Sally, a gainfully employed college graduate, married for 25 years with two grown children. She arrived at the large HMO Addiction Medicine Department, seeking help for her drinking, which by her account, had been a problem for more than 20 years. In the two years I spent working with Sally, she maintained her abstinence, got a sponsor and did her 12 step work. She also attempted suicide three times.

Winter is rapidly approaching and the days are getting shorter. As I gaze out my window on Washington’s Olympic Peninsula, a stone’s throw from the Canadian border, I find myself day-dreaming of the coming Spring and the warmer, sunnier days that will ultimately return. In keeping with my Springtime fantasies, I’ve decided to focus this issue’s column on the topic “What Seeds Are We Planting?” As always, please feel free to share these thoughts with any clients or colleagues who may benefit from this message.
