Closing the Revolving Door: An Intervention to Address Relapse and Readmission

In the past five years, there has been a greater focus on patients who are readmitted to the hospital within thirty days. This is in part because Medicare is paying less for readmissions and in part because providers want to provide the best care possible. Available literature on thirty-day readmissions focuses primarily on general hospitals and not specialized treatment hospitals. However, what is evidenced in the research for all treatment is that good care lowers readmission rates.
